"ASK THE BODY": Doubts and answers about HIV

I love Dr. Bob.

I came across his forum at "The Body" the way most of his numerous fans came across it: worried that I might have exposed myself to the risk of contracting the virus that causes AIDS. I searched the Internet for information on whether my fears were reasonable. Information was to be found all over the place. Dr. Bob’s coolheadedness, humanity, immediacy and incredible sense of humor, on the other hand, were to be found nowhere else. My question was answered a long time ago, and yet I still frequently log onto his forum, and I am one of many to do so! It is packed with information, full of human stories, at times touching, oftentimes laugh-out-loud funny.

Dr. Bob, or to be more official, Dr. Robert J. Frascino M.D., speaks from a unique vantage point: a board-certified allergist/immunologist, one of the first doctors to specialize in AIDS and the virus that causes it – HIV – and at the same time a carrier of the virus himself for almost 16 years now, he has the unique advantage of seeing things “from both sides of the examination table,” as he says himself. He was infected in 1991 from a needlestick accident, which he sustained while he was performing a routine medical procedure on a patient with advanced AIDS. He often says he’s living on “borrowed time,” as at the time of his infection his life expectancy was not more than 6 or 7 years. But he has never given in; he faces the dozens of pills he has to down every day (and all their serious side effects) with unending optimism and humor, and he’s always there to support not only others infected by the virus, but also the misinformed public at large like myself, and even a whole bunch of paranoiacs who touched a stripper’s nipple and their worst fear is getting what Dr. Frascino has been living with for the past 16 years!

The philanthropic organization he owns and runs with his longtime partner, Dr. Steven Natterstad M.D., has raised more than a million dollars for the fight against AIDS. As they both are accomplished pianists, they hold highly successful fundraising concerts; funds are also raised through their connections to pharmaceutical companies, through their organization’s website, and also through Bob’s many fans from his forum at The Body. Their organization also runs a clinic in which, contrary to the HMO’s and the American government’s whims, patients and test seekers are treated with absolute confidentiality/anonymity, compassion, and understanding – there is always someone with you during the 20 minutes you have to wait  for the result of the diagnostic test you just took, and if the result is negative (which is to say, happy), you are not only allowed but rather encouraged to go running up and down the hallway screaming, “WOOOO-HOOOO!!”

In Greece, although we fortunately have free diagnostic tests and, even more importantly, free and universal access to the very expensive, life-saving anti-retroviral medications, when it comes to public information and awareness, be it the sero-positive public or the sero-negative, we unfortunately have a long way to go still. Don’t wonder. Don’t stick your head in the sand pretending it can’t touch you. Don’t avoid getting tested. Drop in to Dr. Bob’s forum – not only will you learn a lot but you will also laugh your head off, and you will draw strength and life inspiration from a magnificent human being. And if you still have questions, don’t hesitate to ask him.

Dr. Bob’s forum at The Body can be found at http://www.thebody.com/Forums/AIDS/SafeSex/index.html.

Dr. Bob,

I am an HIV- man who just ended a relationship with an HIV+ woman. I got tested and was told some interesting things at the government test site.

I was told that unless there are herpes or a significant cut or sore it is impossible to get HIV from vaginal sex. She told me that in 5 years of testing that the only woman to test positive had anal sex. She also said that the CDC is on the verge of coming out saying that there is no risk with vaginal sex unless there is a sore/herpes/cut.

Is this accurate? My fear is that if the CDC does say this different status couples will stop using condoms.

One other note is that she said in 5 years she never once had a positive test from those who used condoms everytime even when having anal sex with HIV+ partners! So that should tell everyone out there that condoms really work.

Keep up your great and imporatant work.

Response from Dr. Frascino

Hello,

The government test site told you what????????

It's "impossible to get HIV from vaginal sex" if you don't have herpes or a significant cut?????? What????

"The only woman to test positive had anal sex"????? What??????

You ask, "Is this accurate?" No, of course not. The vast majority of the world's 40,000,000 cases of HIV were contracted by heterosexual vaginal sex (without herpes and/or cuts!). Just remember this is the same government that told us all about those Weapons of Mass Destruction and Saddam's link to Al Qaeda and 9/11. We all know how accurate and "truthful" that turned out to be.

The really interesting thing in your post was the statement that ". . . the CDC is on the verge of coming out . . . ." WOW! Maybe they saw Brokeback Mountain!?!

Our current government is science-phobic and doesn't really "believe" in facts or common sense. I suggest you stay tuned to The Body and other reputable HIV/AIDS information sites for your HIV information. Tune into the Bush and Cronies' show only for the latest in malapropisms and neologisms! No doubt Dubya gets the gold medal in that competition.

Stay well,

Dr. Bob

I Thought About You In Bed With My Woman, Dr. Bob

(Dec 11, 2005)

Dear Dr. Bob.

This is both a question and a thank you.

Two months ago, I met an incredible woman. To put it bluntly, she's the woman of my dreams. We have not been intimate yet, but two week ago we were fooling around and things got hot and heavy. She told me that she did not believe in condoms, and that I should trust her. Even though I was tempte to have unprotected sex with her, I thought about you while in bed. Let me explain. I thought about all the postings that you answer, and I remember all the people who wrote to you in anguish. I thought about so many people who have HIV and AIDS, and I decided that I would follow you advice in the Forum: that ONLY I am responsible for my sexual health. So, thank you. The question I have is: considering how sensitve this woman is about condoms, "how" is the best way (i.e., senstive way) to ask her to get tested, and let her know that this is not an issue of distrust for me, but one of my health?

We love you, Doc!

Tony

Response from Dr. Frascino

Hey Tony,

"I Thought about You in Bed with My Woman, Dr. Bob!?!?" You can probably imagine the title of your question had me worried for a minute. I was relieved to read the details of your thoughts!

Regarding your dream woman who does not "believe" in condoms, I would suggest a very direct, but caring, approach. HIV/AIDS prevention via safer sex techniques is not a "trust" issue, but rather scientific fact.

Fact: More than 25% of the over 1,000,000 Americans who are HIV positive have absolutely no idea they are HIV infected.

Fact: Latex condoms, when used properly, are remarkably effective in decreasing the risk of HIV and many other STDs.

Fact: Condoms may be inconvenient, but not nearly as inconvenient as having AIDS.

Fact: She may decide she doesn't "believe" in condoms and decide not to use them. However, your decision can be that you do believe in condoms and don't have sex without them.

Finally, advise her that you are going in for your routine periodic HIV-screening test and invite her to come along. Make it convenient for her schedule.

If this dream woman does not accept your decision to protect your health, you should consider the fact that your dream woman might really be a nightmare in the making.

Good luck. Remember you are being perfectly reasonable. Also, I'm flattered you thought of me in bed!

Stay well, Tony. Please do stay well!

Oral in Steam Room

(Jan 3, 2006)

Ok, I am married and am now freaking out. I was at the gym the other day and while in the steam room ended up getting a hand job from the guy beside me and then oral from the guy in front of me. How much of a risk have i put myself into? I am scared to think and don't want to go back to the gym. I have found out it is a very gay gym and all kinds of things go on there.

Response from Dr. Frascino

Hello,

You went to the gym and while in the steam room the dude beside you spanked your monkey and then dude in front of you sucked your one-eyed monster, and subsequently you found out this is a very gay gym???? Wow, what a shocker it must have been to find that out!!! Hmmm . . . didn't that thought cross your mind while you were being manhandled, swallowed and basically pulled simultaneously in more directions than a Prince Albert at an S&M party??? OK, let's just say you may not have a very keen sense for the obvious and we'll move on to your questions about HIV risk. Overall it's very low. Masturbation is not considered a risk for HIV transmission and insertive oral sex carries a very low risk at best. If you remain worried, HIV testing is always an option available to you. However, wait for at least the three-month mark so that your results will be definitive and conclusive.

Now, just because I know many of the readers of this forum will ask, I have a question for you: exactly where is this gym???

Good luck.

Dr. Bob
